Transitioning Initiative (TI) provides a special category of eligibility those aged 14 to 21 and priority for services to ensure a smooth transition from high school to adulthood. We assist in connecting youth with resources and services, ensuring they receive the support needed to achieve their post-school goals, whether it's pursuing higher education, finding employment, or living independently.

For traditional services, participants identify licensed or certified provider agency who can offer assistance with residential housing, day services that can include meaningful activities, volunteering, or even employment-seeking. As well as other and other necessary supports.
Self-directed option give individuals with developmental disabilities more control over their support and services. This approach allows participants to choose the services they need, manage their own staff and services, and have a say in how their care is provided.
